Changing Your Pension Beneficiary (and other things)

Are you a Full-Time Permanent CTA worker? Have you remarried or divorced? Something major changed in your life? Please make sure the right persons are listed as your Pension Beneficiary. Fill out and send the Pension Board a completed Form RP116 at the pension website:

• Qualified Domestic Relations Order (for example, when you have to split pension with a former spouse)
• Change of address, union dues deduction for retirees
